Onsite Reporting

Personnel who will work on campus must fill up this new Onsite Reporting Form.This is to advise the school that you are reporting on site. This is also where you can book your shuttle ride.  Cut-off time to submit the form is 4:00pm the day prior to your arrival.

Take note of safety protocols:

  • Keep your face mask on at all times.
  • Observe social distancing throughout the campus.
  • Be mindful of the signs and follow them.
  • Stay clear of areas that are marked “Do Not Enter.”
  • Wash your hands frequently.
  • Bring your own meals because there is no canteen service.
  • Do not report on site if you have flu-like symptoms (i.e. cough, cold, sore throat, fever, etc.)
